Bacteriological Etiology of Pneumonia in Patients with Tuberculous Destroyed Lung
Background: Tubercuous destroyed lung (TDL) is sequelae of pulmonary tuberculosis and causes extensive destruction of lung and pulmonary function. The common causes of hospitalization are pneumonia, hemoptysis, and acute exacerbation in TDL. However, there are few reports on the bacteriological etiology of pneuomonia in TDL.
Methods: We retrospectively evaluate 162 patients with TDL at the Keimyung University Dongsan Hospital from January 2004 to August 2015.
Results: Pneumonia (132 cases, 39.3%) was the most common cause requiring hospitalization in TDL. Pneumonia etiology was found in 69 cases (52.3%) of the 132 cases. Bacteriological pathogens isolated from pneumonia were in the following oder of frequency, Pseudomona aeruginosa (n=27), Acinetobacter baumannii (n=9), methicillin-resistant Staphylococcus aureus(n=9), Klebsiella pneumoniae (n=8), Stenotrophomonas maltophilia (n=4), Streptococcus pneumoniae (n=3), and Escherichia coli (n=2). During the follow-up period, tuberculosis was reactivated in 24 patients (14.8%) and sputum specimens of 13 patients showed positive cultures for non tuberculous mycobacteria.
Conclucsions: Bacteriological etiology of pneumonia in TDL is different from community-acquired pneumonia. Pseudomona aeruginosa was the most frequent pathogen.
